“I reached out to Nada for guidance on a new discharge education pilot I was planning at my hospital site. It was incredibly helpful to have the input of someone who had successfully implemented transitions of care programs at other health systems. I especially appreciated her advice on common pitfalls in the discharge process.”

At the completion of her PGY-2, Dr. Abou-Karam provided consulting services to the CMS Community-Based Care Transitions Program (CCTP) at Sharp HealthCare, and then joined the faculty at the USC School of Pharmacy, where she served as the Care Transitions Pharmacy Program Coordinator and represented the School of Pharmacy on the CMS/Health Services Advisory Group (HSAG) Care Coordination and Medication Safety Collaborative.
After recognizing the crucial role of ambulatory management in the care continuum, Dr. Abou-Karam returned to Sharp HealthCare where she practiced for two years within Sharp Rees-Stealy Medical group.
Dr. Abou-Karam now lives in Los Angeles, CA with her newlywed husband. She decided to utilize her experiences as a published clinician-writer and successful healthcare quality professional to start NAK Consulting, LLC.
